By completing and submitting this form, You Agree to the following Terms and Conditions: I/We authorize and grant you or any assignee limited power of attorney to make whatever credit and employment inquiries that may be deemed necessary in connection with this application for Credit Repair program with Perfect Credit Repair, Inc. Perfect Credit Repair, Inc is not a "credit repair company". Under Federal Law, it is illegal to make guarantees for "credit repair". The entire Fair Credit Reporting Act is posted on the Perfect Credit Repair, Inc Website. All work will be performed on a "best-efforts" basis with no guarantee on results. I/We agree to hold you and your employees, officers and agents harmless for any claims, suits, actions or demands arising out of this application and will not hold you responsible for any credit rating past, present, or future. I/We have read and understand the terms and conditions of this membership agreement. I/We acknowledge that I/we have three business days to cancel this application. Agreement This Credit Repair Services Agreement (the "Agreement") is entered into and effective today (the "Effective Date") by and between Perfect Credit Repair, Inc., a Florida corporation ("PCR"), and the client. (“client”). Payment Schedule The client agrees to pay $75.00 by Check, Money Order or with a Credit Card on a monthly basis for 12 months. This “payment plan” will be effective beginning on the date that the first payment is received. Power of Attorney The client hereby grants PCR limited power of attorney to make whatever credit inquiries are deemed necessary in connection with this application. PCR is not a “credit repair company”. Under Federal Law it is illegal to make guaranties for credit repair. All work is performed on a best effort basis. The client is to hold PCR harmless for any claims, suits, actions, demands arising out of this application. The client acknowledges that they have read and understand the terms of this application and acknowledge that they have five (5) business days to cancel this application in writing. The client agrees to follow any and all reasonable directions from PCR regarding actions to take to raise their credit score. The client understands that if they fail or refuse to follow those directions, their credit score may not improve and PCR will not be held responsible. All monies are deposited into an escrow account until all services are performed.
